The Extended Team

Community Nurses

Our Community Nursing Team travels to the patients own home and local residential homes, assessing the nursing needs and providing care to housebound patients who are unable to travel to the surgery.

Referred patients can contact the community nurses by telephone on 01822 617602.  There is an answerphone which is checked regularly throughout the day if they are unable to answer your call.

Health Visitor

The Health Visitor Team will be based at Tavyside Health Centre and can be contacted on 01822 616135.

Health Visitors are qualified nurses with further specialist training, who are experienced in child health surveillance, health promotion and health education. They work with the families of the 0-5 age group across the surgery population.

Midwives

The Midwifery Team will be based at Tavyside Health Centre and can be contacted on 01822 616953.
Chris Pyle, our midwife, holds a clinic here every other Wednesday morning.  She works alongside the doctors and Health Visitor.

Chiropody

All of our chiropody/podiatry referrals are sent to Seventrees Clinic in Plymouth.  Clinics are available at Tavistock Clinic for patients who find it difficult to travel to Plymouth.

Physiotherapy

If you need to see a physiotherapist we will refer you to Tavistock Hospital.  One of the team will contact you directly to make an appointment.
Counsellor

The surgery offers a counselling service, which can be accessed by referral from your doctor. Our counsellor is registered by UKRC and is accredited by the British Association for counselling and psychotherapy. She works to the BACP ethical framework for good practice.

Macmillan Nurse

Lillian, our Macmillan Nurse, gives support and comfort to terminally ill patients and their carers.

Lillian is in regular contact with St Luke’s Hospice and can advise doctors and patients alike on advances in care of the terminally ill.

Dietitian

Our Community Dietitian is responsible for running a Dietetic Clinic at Estover Health Centre by referral from your doctor. Dietitians interpret the science of nutrition in a practical way providing information and advice on ways of changing eating habits to treat medical problems to prevent disease and promote nutritional health.

Key areas of work include diabetes, coronary heart disease, being underweight, digestive problems, food allergy/intolerance, eating disorders, consistency alteration, nutritional assessment, and obesity.

Referrals to a Consultant via Sentinel Healthcare

If during your consultation with the GP it is suggested that you need to be referred to see a Consultant for further investigations, this would generally be done through Sentinel Healthcare. This is a very valuable service which allows people much more choice and flexibility.

Once your doctor has decided to refer you, he/she should give you an information leaflet which will have all the details you need concerning your referral.  He/she will insert your NHS number onto this document should you need to contact Sentinel at any point – hopefully this won’t be the case!

Sentinel will call you within five days of your referral offering you the next available appointment convenient to you and a choice of locations.  Please make sure that we have all your current details to enable Sentinel to contact you as speedily as possible.
