Summary Care Record (SCR)

The SCR is an electronic record of important patient information, created from GP medical records. It can be seen and used by authorised staff in other areas of the health and care system involved in the patient's direct care.

The NHS Summary Care Record has been introduced to help deliver better and safer care and give you more choice about who you share your healthcare information with.

What is the NHS Summary Care Record?

The Summary Care Record contains basic information about: 

  • Allergies and details of any previous bad reactions to medicines
  • Current medication
  • The name, address, date of birth and NHS number of the patient

The patient can also choose to include additional information in the SCR, such as details of long-term conditions, significant medical history, or specific communications needs.

The intention is to help clinicians in Emergency Departments and ‘Out of Hours’ health services to give you safe, timely and effective treatment. Clinicians will only be allowed to access your record if they are authorised to do so. You will always be asked for your express consent before a healthcare professional can look at your Summary Care Record, unless it is an emergency in which case they can look without your consent, for instance if you are unconscious. You can refuse if you think access is unnecessary. 
Over time, healthcare professionals treating you may add details about any health problems and summaries of your care. 

Children under the age of 16 

Patients under 16 years will have a Summary Care Record created for them unless their GP surgery is advised otherwise. If you are the parent or guardian of a child then please either make this information available to them or decide and act on their behalf. Ask the surgery for additional forms if you want to opt them out. 

  • If you are happy for a Summary Care Record to be set up for you then you need take no further action. 
  • If you wish to opt-out of the summary care record, please click here, complete the form and return it to us. 

Local Shared Health Record 

Tavyside Health Centre are now live with the Local Shared Health Record initiative.  This will mean that other local healthcare professionals will be able to see your medical records when they are providing you with care.  All patients have the choice of opting out of this initiative.  

To opt in or out please click here to complete the necessary form and return it to the surgery.
