Community Nurses

Our Community Nursing Team travels to the patients own home and local residential homes, assessing the nursing needs and providing care to housebound patients who are unable to travel to the surgery.

Referred patients can contact the community nurses by telephone on 01822 617602 for the Tavistock Team and 01566 784801 for the Lifton Team.  There is an answerphone which is checked regularly throughout the day if they are unable to answer your call. 

Health Visitor

The Health Visitor Team are based within the Children's Centre just next door and can be contacted on 0333 234 1901. 

Health Visitors are qualified nurses with further specialist training, who are experienced in child health surveillance, health promotion and health education. They work with the families of the 0-5 age group across the surgery population. They also work in conjunction with the School Nurses who can be contacted on the same number.

Midwives

The Midwifery Team will be based at The Children's Centre just next door and can be contacted on 01822 619711.

Podiatry

The GP can refer you to a local podiatry service.  Where you are seen will depend on where you live.    

Joanne Whiteley also carries out a private clinic at the Tavistock surgery on a weekly basis.  If anyone wishes to use this service you can obtain her details at the reception desk.

Physiotherapy

If you need to see a physiotherapist you can be seen in either Tavistock Hospital, Launceston Hospital or Derriford Hospital depending on where you live.  You can self-refer for this service.  Just ask at Reception for a physiotherapy self-referal form. 

Counsellor

The surgery offers a counselling service, which can be accessed by referral from your doctor. Our counsellor is registered by UKRC and is accredited by the British Association for counselling and psychotherapy. She works to the BACP ethical framework for good practice.

Depression and Anxiety Service 

The Depression and Anxiety Service is a service for people in the South and West of Devon who are 18 years and over who are feeling stressed, anxious, low in mood or depressed.  You can self-refer to this service simply by calling 01626 203500. 

St Lukes Specialist Palliative Care Nurses

Derek Hart is our Specialist Palliative Care Nurse. He gives support and comfort to terminally ill patients and their carers. 

He is in regular contact with St Luke’s Hospice and can advise doctors and patients alike on advances in care of the terminally ill. 

Dietitian

Our Community Dietitian is responsible for running a Dietetic Clinic at either Estover Health Centre or Derriford Hospital by referral from your doctor. Dietitians interpret the science of nutrition in a practical way providing information and advice on ways of changing eating habits to treat medical problems to prevent disease and promote nutritional health.

Our Dietitian sees patients of all ages from antenatal through to children, adults, and the elderly. Key areas of work include diabetes, coronary heart disease, being underweight, digestive problems, food allergy/intolerance, eating disorders, consistency alteration, nutritional assessment, and obesity.

Referrals to a Consultant Devon Referral Support Services (DRSS)

If during your consultation with the GP it is suggested that you need to be referred to see a Consultant for further investigations, this would generally be done through DRSS. This is a very valuable service which allows people much more choice and flexibility.

Once your referral has been submitted you will receive a letter from DRSS within five working days which will contain all the information you require to book your appointment.  Please note, not all referrals require an actual appointment with a consultant.  Services are constantly being updated and a number of specialities offer verbal advice to the GP which they can then duly relay to you.  

The DRSS require a blood pressure reading, height and weight measurement and smoking status for all referrals.  All these measurements must have been taken within the last 12 months.  If your blood pressure has not been taken for a while please use the machine in the reception area if you know you are going to be referred.
